Online Referral Form

Restore’s referral form is now online!  You can still access our downloadable referral form, but we also have a form that you can fill in directly on our website.

Do you know somebody who you think would benefit from Restore’s befriending service or group social activities?  You can refer somebody to us, and we’ll be in touch with them to give them more information about how we can help them.

Click here to use our online referral form.
